THE IMPORTANCE OF ARTERIAL STIFFNESS ASSESSMENT IN PATIENTS WITH GRADE 1 HYPERTENSION

Abstract

Assessing the arterial stiffness (AS) in hypertensive patients brings complimentary information for the cardiovascular (CV) risk stratification, but current data is rather limited in mild hypertensive patients. The purpose of the study was to evaluate the association between CV risk factors and AS markers in patients with grade 1 hypertension. Material and methods: The study was prospective, conducted over a 14-month period and included 58 patients diagnosed with grade 1 hypertension. Patients were extensively analyzed for the presence of CV risk factors and for the determination of AS parameters – pulse wave velocity (PWV) and aortic augmentation index (AixAo). Results: Mean age was 63.98 ± 12.24 years, 38% males. PWV was directly associated with age (r = 0.27; p = 0.03), total cholesterol (r = 0.33; p <0.01), LDL-cholesterol (r = 0.25; p = 0.05), SCORE risk (r = 0.25; p = 0.05) as well as with the presence of stable angina (r = 0.31, p = 0.02). AixAo was associated with age (r = 0.35; p = 0.01), systolic BP (r = 0.51; p < 0.01) and diastolic BP (r = 0.37; p < 0.01). Moreover, AixAo was positively associated with total cholesterol (r = 0.46; p < 0.01) and LDL-cholesterol (r = 0.36; p = 0.01). Conclusions: In patients with grade 1 hypertension, arterial stiffness markers are directly associated with age, lipid parameters, BP and SCORE risk. Therefore, it is essential to apply early specific therapeutic measures to control the CV risk factors in this group of patients.
