EVALUATION OF THE NEURO-AFFECTIVE STATUS IS A DECISION FACTOR IN THE THERAPEUTIC APPROACH OF SENIOR PATIENTS WITH HIP FRACTURES
Abstract
Both the cognitive function and the affective status are fundamental geriatric cues in the objective and comprehensive evaluation of a senior patient. Aim: To investigate the impact of cognitive and emotional disturbances of senior patients with hip fractures over the postoperative prognostic. And recovery period. Material and methods: We assessed the cognition and affective status as well as the degree of fragility in group of 44 senior patients admitted in the Trauma Clinic, Iasi for hip fracture. The patients underwent preoperatively a complete geriatric evaluation, focusing on the assessment of the cognitive function (MMSE - Mini-Mental State Exam), psycho-emotional status (GDS-Geriatric Depression Scale) and fragility (Groningen-IGF fragility index). Results: Most of the evaluated patients were female, and over 80 years old. Moderate cognitive impairment was approximately 40% in the studied group, 27% associating fragility. The mild depressed patients, with fragile status, accounted for more than a half of the percentage of those analyzed. The patients with cognitive and affective disorders registered a negative prognostic on the postoperative evolution and during the recovery period. Conclusions: The results of this study confirm the need for a complete geriatric evaluation in senior patients with hip fracture, or at least the evaluation of cognitive and emotional status associated with the degree of fragility as they are considered important prognostic factors in the post-surgical and recovery period.
